If you are interested in hosting a tournament in 2012-2013, then fill out the form below and e-mail it back to Kevin Berlat at berlat@phoenixunion.org by Monday, April 16.  Please include your preferred date, and at least one alternate date in case of a calendar bunch-up.  Also provide a number where we can reach you during our planning meeting in case we need to talk about dates.  Our planning meeting is on April 18th.
